STUDENT APPLICATION FORM
CLASS SCHEDULE
Beginning and ending dates for twelve week session.
August 23, 2010 to November 12, 2010
January 3, 2011 to March 25, 2011
April 4, 2011 to June 24, 2011

The two, four, and six week sessions are only offered in the April and August sessions which run concurrently with the twelve week session. These sessions start every Monday which will begin and end within the beginning and ending dates of the twelve week session. If the session you would like to attend is full, we will apply your $100.00 deposit to your second choice or refund your deposit. Also, you may change your classes or have your deposit refunded providing the school is notified in writing 4 weeks prior to the start of class.
If you change your mind about attending this school after the 4 weeks prior to the start of class or fail to pay the balance when due, you will forfeit your $100.00 deposit and your $50.00 housing deposit.
HOUSING - (if needed) A $50.00 deposit is required with the application to reserve housing and cover damages or clean up cost. This deposit is refundable upon inspection of the property. Refund should be expected in 4 weeks upon passing inspection of the property. The housing balance will be due first day of class.

BASIC FOUR WEEK SESSION– (140 hours)                                     TUITION        $1600.          HOUSING   $300.
Choice of class dates: 1st_________ 2nd_________ Enclosed is my $100.00 deposit. I will pay the tuition balance of $1500.00 on the first day of class.
INTERMEDIATE SIX WEEK SESSION–(210 hours)                          TUITION        $2200.          HOUSING   $400.
Choice of class dates: 1st_________ 2nd_________ Enclosed is my $100.00 deposit. I will pay the tuition balance of $2100.00 on the first day of class.

All 2,4,& 6 WEEK SESSIONS WILL START EVERY MONDAY AND WILL RUN CONCURRENTLY WITH THE 12 WEEK SESSION

ADVANCED TWELVE WEEK FARRIER SESSION–(420 hours)     TUITION       $3600          HOUSING  $700.
Choice of class dates: 1st_________ 2nd_________ Enclosed is my $100.00 deposit. I will pay the tuition balance of $3500.00 on the first day of class.
